I wish to formally notify you that I am resigning from my position of (Job Title) with (Company name). My last day of employment will be the (enter your last employment date here), as per my terms and conditions of my employment.
As you are aware, I have been on Maternity Leave for the past eight months, since the birth of my Daughter. Due to these circumstances, I have taken the decision not to return to work until she is older, so I can spend these important early years at home with her.
The terms of my employment contract state that I am required to give you four weeks’ notice; therefore my date on which I believe my maternity leave and employment to terminate will be (insert date).
