With effect from the above date, please accept this letter as notification that I wish to resign from my position as (Job Title) within (Company).
It is with a regret that I leave this position, but due to on-going ill health, I find it is no longer possible to continue my duties with the company.
I believe I am required to provide you with four weeks’ notice of my resignations, and therefore my departure date will be (insert date). As I am currently on sick leave, it is my understanding that I am not eligible to use any of my accrued holiday entitlement during the notice period, but am entitled to it as paid in lieu. Please advise me if this will be paid to me and, if so, when.
